OfficeZilla Return Authorization Form

As soon as you give us some details, below, we will process your request. We will send along a UPS packing slip to return the item. You can arrange a pickup (calling 1-800-PICK-UPS makes it easy), give the package to any UPS driver, or take the package to a UPS store.

You must provide the Return Authorization Number on the shipping label to us, or you will not be refunded.

Return Authorizations provided through this form should include the Return Authorization number, but please check with us at returns@officezilla.com before shipping products back to us.
